CCL-managed buildings are designed to provide individualized services to older or disabled adults in a setting that reflects the residential apartment style living of the community in which they are located. The atmosphere at each residence provides for privacy and independence while also creating a sense of community among the tenants, staff, and volunteers.

Caring and qualified staff at each building is dedicated to meeting both the health and social needs of each tenant. Services and activities are available to maintain or improve the capabilities of each tenant, with an emphasis placed on the tenants' abilities rather than their disabilities.

Generally, CCL-managed buildings offer studio or one-bedroom units with private baths and compact kitchenettes including a smaller refrigerator and microwave unit. Units are typically unfurnished so tenants can bring their own furnishings and personal belongings however, assistance may be available in furnishing units for those tenant's otherwise unable.

Common areas in buildings generally include dining room, community/activity room, and various indoor & outdoor sitting areas for socialization and recreation. The majority of buildings also include a library area, living room, outdoor courtyard and garden area, hair care salon, as well as several that offer exercise rooms.

All buildings are equipped with one or more emergency call systems to enable tenants to immediately contact staff from their units, as well as from common areas. Staff is on-site 24/7 at our licensed facilities assuring that services and assistance is available to tenants around the clock.

Generally, cable TV is provided for in the building's community space and public phones are made available for tenant use. Individual units are further equipped for cable access and telephone hook-up, as well as having individual heat thermostat, and smoke alarm. Security systems and controlled access on outside doors of buildings provide additional security for tenants.

Housekeeping, Laundry, and Maintenance Services

All common areas and tenant units are cleaned by staff on a regular basis. Typically, tenants arrange for their units to be cleaned once a week, including changing the bed, vacuuming, and cleaning the bathroom and kitchen. If desired or needed, more frequent cleaning may be scheduled.

Personal laundry and linen service is typically provided to tenants on a weekly basis, with the laundry picked up by staff on a regularly scheduled day and returned the following day. Laundry service may be provided more frequently if needed or desired. If the tenant or family member wishes to launder the items, washers and dryers are available for use at no charge.

Building maintenance staff provides all upkeep and needed repairs of the building and grounds. Unit repairs, changes, or additions are accomplished via individual arrangement and maintenance request.

Meal Services

Tenants are typically expected to go to the dining room for meals; mealtime reminds and escorts are available. During periods of illness, meals can be delivered to tenant's units.

The main objective of our meal services is to provide attractive, nourishing meals that meet both tenant preferences and any physician's orders they may have (e.g. for a special diet). A registered dietitian is responsible for developing the menus at each facility and ensuring that all meals served meet USRDA guidelines. Tenants are encouraged to participate in the menu planning process through tenant food committees.

Three meals daily are generally served restaurant-style in a main dining room, with full waitress service. The menu selection for meals is made available to tenants in advance. Tenants are encouraged to express their food preferences, and may discuss any concerns or alternative food preferences they may have regarding the menu selection with the Director.

Snacks and other nourishments are also available and orders for special dietary supplements (e.g. Ensure) may be coordinated.

Personal Care and Medical Services

Personal service assistance is available 24 hours a day to provide tenants with the necessary services and security to ensure tenants. needs are effectively met. Tenants may receive assistance with monitoring health care, ordering supplies/supplemental services, arranging medical care, medication administration, bowel and bladder management, night time care, personal hygiene, bathing, dressing, ambulating, eating, orientation, communication, counseling, behavior management, socialization, monitoring of safety, and transportation.

The intensity and frequency of services received by each tenant will be reviewed quarterly with the involvement of the tenant or Authorized representative, tenant's case manager, family, and facility staff.

Nursing related services shall be available as permitted by state licensure law. Limited Nursing Services to include routine assessment, health monitoring, and other nursing tasks as requested by the tenant and as required/permitted through regulation.

For additional health-related services such as home health or private duty assistance, building staff may be available to assist tenant in coordinating and privately obtaining such services, where appropriate. Staff is also available to assist tenant in scheduling of outside medical appointments and coordinating necessary transportation to these visits.

In the event of acute or emergency medical care need staff is available to make arrangements for hospital transfer via ambulance or private transport including notification to physician and family.

Special personal services such as transportation, trust accounts, mail screening, shopping, and pet care are also generally available.

Other Services

Social, recreational, educational, and wellness programming are added areas of focus in CCL-managed buildings. We strive for well-balanced programs that provide opportunities for participation in social, recreational, intellectual, physical, spiritual, and service activities. Scheduled activities and special events are typically adapted to the interests and needs of each community with an active tenant council participation present.

In-house wellness groups, exercise programs, and strength training; visiting speakers, musicians, and clubs; standard bingo, crafts, and gardening; community outings, neighborhood activities, and special events planning may all be found in our communities. Individual buildings should be contacted for detailed listings of activities available at their community.

Service Philosophy

It is the goal of each CCL-managed facility to provide residents with those services they want and need in a manner that supports their individual lifestyle and preference. To facilitate this goal, a personalized Negotiated Service Agreement is developed with the resident, and if they choose, others representing them. Each Negotiated Service Agreement provides a detailed record of those services that the resident would like the staff at the facility to provide. The agreement is then reviewed and revised quarterly and/or as needs and preferences change.

If resident preferences could potentially place them or others at risk, a Managed Risk Agreement may also be developed. The purpose of this agreement is to provide a structured opportunity for the resident (and family or other representative if desired) to discuss with the Director the possible risks associated with the resident's preferences. At that time a plan that would minimize the potential risk(s) would be developed and agreed upon and be incorporated into the Negotiated Service Agreement.

Individual buildings should be contacted for more detailed information relating to move-in criteria or for specific conditions, which may require move-out based upon state regulations and/or funding requirements. Building-specific tenant handbooks, rental, and service agreements are available.
